Where can I get an SR50 auto insurance form that I need to fill out?

An SR50 is a form that a driver used to show proof to the Indiana Bureau of Motor Vehicles (BMV) that the driver has the current insurance that shows the start and end date of the current insurance policy.
You may be able to get Affidavit of Current Insurance “SR-50″ form online through the Indiana government website. You may print it out. Once you got your form printed out, be sure to completely fill it out so it will save you time when BMV office. Besides the personal information part on the form, you will also have to have your car insurance agent or car insurance company fill out the policy information to make sure that the policy information is correct.
There is also a part on the form that requires signature of an authorized representative from your insurer. The IN BMV also states that any fraudulent signature on the SR50 insurance form will be subject to driver license suspension.
